TITLE: Nurse Practitioner (NP) Care Manager
REPORTS TO: Director of Care Management
DEPARTMENT: Care Management
FLSA STATUS: Exempt

JOB SUMMARY:
The purpose of this position is to conduct in‐home assessment of high‐risk patients that includes evaluation of physiological, psychosocial, environmental, financial, and health‐related behavior. This position will primarily perform field work outside of the office environment.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
1. Establishes a plan of care with patients, providers, and family that identifies and continuously reassesses opportunities for cost‐efficient appropriate levels of care.
2. Enhances communication and collaborative relationships with multidisciplinary healthcare team members.
3. Directs field assessments for ancillary team members such as RN's, LPN’ and Social Workers for assigned members in a multidisciplinary team approach.
4. Emphasizes continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan.
5. Acts as a patient advocate protecting privacy and confidentiality issues.
6. Provides patient education, monitoring of health needs, and coordination of community resources.
7. Prevents adverse patient occurrences when possible and intervenes quickly if prevention is not possible, thereby minimizing poor outcomes.
8. Facilitates patient empowerment and quality of life by promoting educated independent patient choice on all aspects of care.
9. Maintains a comprehensive working knowledge of community resources, payer requirements, and network services for managed members. Identifies opportunities for health promotion and illness prevention
10. Collects relevant patient data to support VIVA Health star metrics and quality initiatives.
11. Conducts required assessments and interventions within time frame specified and documents activities in Pragmate in a timely manner.
12. Work expectation consists of up to 75% field based activities conducting assessments with members with remaining 25% in VIVA corporate office.
13. Performs other duties as assigned or needed.
MINIMUM QUALIFICATIONS:
EDUCATION:
- Licensed Nurse Practitioner/Advanced Practice Registered Nurse in a good standing with the State of Alabama.
- Master's degree in Nursing from an accredited college or university.
EXPERIENCE:f experience required and/or preferred
- Minimum of 5 years of current clinical nursing experience with a preference for:
- Two years independent field based clinical experience and/or case management
- Physician Office assessment with coding/comprehensive review of members
LICENSES/CERTIFICATIONS/REGISTRATIONS:
- Current APRN Licensure in the state of Alabama
- Maintains current CPR certification
- Valid Alabama Driver’s License
- Must have reliable means of transportation and maintain automobile insurance and registration according to HR policy.
KNOWLEDGE/SKILLS/ABILITIES:
- Good computer data entry skills.
- Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management.
- Highly organized, detail oriented and skilled at multi‐tasking.
- A team player, strong interpersonal skills, flexible and a positive attitude.
- Ability to exercise confidentiality, discretion, diplomacy, professional poise and appearance.
- Excellent written and oral communication skills.
PHYSICAL REQUIREMENTS:
- Must be able to drive to remote areas
- Must bend, stoop, and kneel frequently
- Must be able to work under pressure from deadlines and goals

- Complete all company required training, job specific or departmental training, as well as any training needed to maintain licenses.
