Dear Nursing Colleague,
My name is Deborah (Deby) L. Lee. I am a student in the Doctor of Nursing Practice program at the University of Alabama in Huntsville. For my scholarly project, I am conducting a survey to gather information on nurses' involvement on health care governing boards and the perceived facilitators and obstacles to serving on boards. The survey is entitled, "Participation of Alabama Nurses on Governing Boards: Identification of Facilitators and Barriers". The survey is only for Registered Nurses in Alabama.
I am requesting your participation in this survey.
The Institute of Medicine (IOM), The Future of Nursing: Leading Change, Advancing Health, proposed that more nurses participate in governing boards. This survey will provide baseline information that can be used by organizations to enhance participation by nurses in health care governing boards.
Aggregate survey results will be disseminated in nursing publications and to nursing organizations including the Alabama Health Action Coalition (AL-HAC). The AL-HAC is Alabama's team for the Robert Wood Johnson Foundation (RWJF) Campaign for Action. The purpose of the coalition is to implement actions identified in the IOM report in order to advance nursing.
Participation is strictly voluntary; participating in the survey implies consent. The study has been approved by the Institutional Review Board of the University of Alabama in Huntsville and can be completed in approximately 10 minutes. Data will be reported in aggregate so no individual or organization will be identified. The individual survey results will be anonymous.
After you submit the anonymous survey, there will be a second link to submit an email address enabling you to receive the survey results and to register for a drawing for one of two $50 gift cards to be awarded. The registration will not be connected to the survey in any way. Email addresses of participants in this part of the project will be forwarded to the AL-HAC in order for progress reports of the Coalition to be quickly and economically disseminated to Alabama nurses.
You may elect not to participate in this part of the project. Your anonymous survey responses will be counted whether or not you provide your email address. Any questions can be forwarded to the principle investigator, Deborah L. Lee, MSN, RN, DNP student at UAH, at firstname.lastname@example.org

By participating in this study, you agree that you are 19 or older. This study was approved by the Institutional Review Board at UAH and will expire in one year from April 24, 2014.
